Documentation that works with you,
not against you

Saba is built around a simple idea: the information that never changes about your patient should never need to be typed again.

1
First time only
Create your patient profile

The first time you document a patient in Saba, you enter their complete profile — everything that stays the same shift after shift. Saba stores this securely and auto-fills it on every form you ever start for this patient.

Saved to profile
Never re-enter
Name · DOB · Medi-Cal # · Diagnosis · History · Formula · Trach type · Equipment · Positioning orders · Tube care
Enter each shift
Always fresh
Shift date · Shift time · Nurse name · Vitals · I&O values · Secretion details · Pain assessment
2
Start of every shift
Open today's form — already half done

When you start a new form, Saba pre-fills every static field from the patient profile automatically. Clinical assessment sections — Neurological, Cardiovascular, Respiratory, GI, Mobility — are pre-loaded from the previous shift's documented values, ready to confirm or update.

Pre-filled from last visit
Confirm or update
Neurological status · Heart tones · Respiratory pattern · Abdomen · Bowel sounds · Mobility/gait · ADL level · ROM · Repositioning orders · Daily schedule routine
3
During the shift
Tap "No change" or enter what's different

For every clinical section, you see a "No change from last visit" button. One tap confirms the entire section unchanged — covering up to 14 fields at once. If something has changed, tap into that field and update it. Most shifts, you'll use "No change" on 4–5 sections and only enter the daily values.

✓ No change
One click, whole section confirmed
Available on: Neurological · Head · Cardiovascular · Respiratory · Gastrointestinal · Genitourinary · Mobility · Daily schedule routine. Saves an average of 30–45 minutes per shift.
4
End of shift
Export or submit in one tap

When your form is complete, export a clean PDF — formatted to match the Registered Nurse Assessment of Medical Necessity your agency and payers already know. Pro and Agency users can submit directly to Medi-Cal and other payers without leaving the platform.

Every field has a purpose

Questions

Common questions

Is Saba compatible with your current clearinghouse? +
Yes. The PDF export from Saba matches the format of the Registered Nurse Assessment of Medical Necessity used on the your current clearinghouse platform. Plus and higher plans export in your current clearinghouse compatible format — so you can upload directly into your current clearinghouse without reformatting.
Is the free plan really free forever? +
Yes. The free plan gives you one patient profile and full access to all three pages of the RN Assessment form forever. The limitation is that PDF exports include a Saba Health Hub watermark, and direct payer submission is not available. You can upgrade at any time to remove those restrictions.
Is Saba HIPAA compliant? +
Saba is built on HIPAA-eligible infrastructure with end-to-end encryption, secure authentication, session timeouts, and full audit logging. Agency plan customers receive a signed Business Associate Agreement (BAA). We recommend consulting with your own legal or compliance team to confirm requirements for your specific use case.
Can parents and non-nurses use Saba? +
Absolutely. Saba has role-based views — nurses see the full clinical assessment, while parents and family caregivers see a simplified daily record view with plain-language guidance. The same patient profile powers both experiences. You select your role when you create your account.
Does Saba work on mobile? +
Yes. Saba is fully responsive — every form, every section, every field works on a phone or tablet. Complete a form at bedside on your phone, review it on your desktop. A native iOS and Android app with offline mode is on our roadmap for Phase 2.
What forms does Saba currently support? +
At launch, Saba supports the Registered Nurse Assessment of Medical Necessity — the 3-page form used on your current clearinghouse for Medi-Cal home care. This includes the clinical assessment, intake & output, vital signs, daily schedule, and nursing notes. Additional forms (medications, intake/output standalone) are coming in Phase 2.
